Certificate Issuance Notice
- Applications via telephone, email or fax are not accepted.
- Certificates will be issued under the name used while enrolled at the school.
- If the documents are incomplete or the fee is insufficient, the certificate cannot be issued.
- If you are a graduate of Osaka Sangyo University Dental Hygiene College, please apply via the application page for certificates, etc. for Osaka Sangyo University University Dental Hygiene College on the School Juridical Person Osaka Sangyo University website.
- If you have any questions regarding the "Certificate of Academic Ability (Certificate of Credit Acquisition for Teaching License)", please contact Center of Teacher Education.

Applying via the Online Certificate Application System
The online certificate application system has been introduced from July 1, 2022. This system is available to Osaka Sangyo University graduates, graduate Osaka Sangyo University Graduate School graduates, Osaka Sangyo University Junior College graduates, and those who have been expelled or dropped out. Current students and those on leave of absence cannot use this system.

If you require any other certificates, please apply by mail or in person at the office.
If you have to have a representative apply for you, please apply at the counter or by mail. The online certificate application system cannot be used.
Regarding the "Certificate of Academic Ability"※1, Please contact Center of Teacher Education.
*1 "Certificate of academic ability" refers to a certificate that calculates the number of credits required to obtain an educational license (hereinafter referred to as "teacher license") in accordance with the classifications set out in relevant laws and regulations.
The main cases where a certificate of academic ability is required are as follows:
① Meet the requirements for obtaining a teaching license and apply for a teaching license to the Board of Education
② Since you do not meet the requirements for obtaining a teaching license, check with the Board of Education to see if you are missing any credits
3) You do not meet the requirements to obtain a teaching license, so you will need to submit this to the university where you are taking courses in order to earn the necessary credits.
How to use the online certificate application system
- In addition to the certificate issuance fee (see separate table) and the certificate return fee (including express delivery fee), a system usage fee of 400 yen is required each time.
- You can apply for up to 20 tickets per application.
- Payment methods are credit card*2 convenience store Pay-easy.
*2 You or a relative must have a credit card (VISA, MasterCard, JCB, American Express). If you are applying from outside Japan, payment must be by credit card.
- You will need to upload an image of your official identification document*3 via the Internet to verify your identity.
- Once payment has been completed, the certificate issuance fee, certificate return fee (including express delivery fee), and system usage fee will not be refunded under any circumstances.
- Receipts cannot be issued.
- The addressee must be the person in question.
*3 Official identification: A valid driver's license issued in Japan (both sides), passport (photo page and holder name section), Japanese-issued health insurance card or qualification confirmation certificate (insured person symbols, numbers, etc. must be masked). Please consult us in advance if you are using any other identification.
Each certificate will be sealed and sent in a return envelope. Japanese certificates will be sent within 5 business days of payment completion. 球天下体育 certificates will be sent approximately 10 business days of payment completion. Please apply with plenty of time. The office is closed and does not issue certificates on Sundays, national holidays, summer and winter holidays, and our founding anniversary (November 1st). Certificates will be created and sent in the order they are created once operations resume.
